This modified enrollment form provides only the basic information to begin the enrollment process for students new to the Sauk Prairie School District. It is necessary to provide supporting documentation to verify your child's legal name and date of birth, proof of residency, and immunizations before the pre-enrollment is complete and the entry in Skyward can occur.  Additional information specific to the 2026-27 school year, such as adding/updating emergency contacts, health information, arrival and dismissal transportation plans, photo and field trip permissions, etc., will be collected during the online Annual School Registration process July 15-31, 2026.
